Hours

Monday through Friday from 9 a.m. to 5 p.m.
We are closed for lunch from 12 noon to 1:30 p.m. There is parking in the rear of the building.

Scheduling Appointments

We are pleased to accept new patients by appointment. If your condition requires prompt attention you will be seen the same day. Appointments are usually made at the patient's convenience and in most cases within 48 hours of your call. Call 518 377-1154 today to schedule your consultation.

Insurance

Dr. Fort is participating provider with MVP, CDPHP, Empire Blue Cross/Blue Shield, Blue Shield of Northeastern NY.

The majority of insurance companies in the U.S. consider the treatment of varicose veins or spider veins to be "cosmetic" and are not covered. If Dr. Fort is not a provider of your insurance company, we will charge you a $100 fee for the initial vein consultation (which may include a diagnostic Ultrasound exam). If you have any vein procedure the initial vein consultation will be applied against the cost of your vein treatment.
